Purpose:

Paper to follow explaining transition from Kirklees smoke-free service to Kirklees Health and Wellbeing model. From 1 April 2017, after the end of the current smoke-free contract, our approach will move into primary care to be delivered by GPs and practice staff. From 1 April 2018 the Kirklees wellbeing model currently being developed will manage our approach, working closely with primary care and public health.

Decision:

That the transition towards a Kirklees Wellbeing Integrated Model, including not renewing the current contract for the Kirklees Smoke-free service, and designing a primary care based approach to commence on 1 April 2017, be approved.
