Transition to Kirklees Wellbeing Integrated Model and Redesign of Smoking Cessation services

Paper to follow explaining transition from Kirklees smoke-free service to Kirklees Health and Wellbeing model. From 1 April 2017, after the end of the current smoke-free contract, our approach will move into primary care to be delivered by GPs and practice staff. From 1 April 2018 the Kirklees wellbeing model currently being developed will manage our approach, working closely with primary care and public health.
